Parishioner Requirement: To register your child for our faith formation programs, you must be a registered parishioner. If you need to register as a parishioner at St. Catherine of Siena and St. Agnes Parish, please do so HERE

1. Complete the Faith Formation registration form HERE
2. Make the requisite payment HERE
3. Email your child’s baptismal certificate to Erin Teske
4. Sign the Diocese of Bridgeport Annual Consent Form (we are currently waiting to receive from the Diocese an updated version of this form, so stay tuned).
